What I collect
The inquiry form asks for your name, your town, a phone number or email address, and a short note about what you'd like help with. The form is for general questions and scheduling, not medical intake.
You can also contact me by email, phone, text, Instagram, or Facebook. Those providers process communications under their own privacy policies. Please do not send sensitive health information through this form, ordinary email, text, or social media messages.
How I use it
I use inquiry information to answer your questions, arrange your complimentary call, and coordinate sessions. I do not sell inquiry information or use it for unsolicited marketing. Service providers need to process information to deliver these services; this is different from selling your information.
Who processes inquiries
The form submits to FormSubmit, which processes the fields and emails the inquiry to my business inbox. My business email is hosted through Microsoft 365 provided by GoDaddy. These service providers may process or retain information under their own policies. FormSubmit may also present a spam-verification step. Security and sensitive information
The website and FormSubmit endpoint use HTTPS to encrypt browser connections. HTTPS does not establish end-to-end email encryption or guarantee that a website or its vendors are HIPAA compliant. The footer phrase “Trusted site” describes this site's HTTPS connection and privacy choices; it is not a seal from an independent certifying organization.
The Stretch Boutique provides wellness and movement services. This website form is not a medical-record system or a verified HIPAA-compliant intake channel. Please do not submit diagnoses, medication details, medical records, or other sensitive health information. If you need to share such information, contact me first to arrange an appropriate way to do so.
Optional analytics and your choice
Google Analytics loads only after you choose “Allow analytics.” If you decline, Analytics does not load, and the inquiry form, phone, text, email, and social links still work. With permission, Analytics uses cookies and technical information to report page views and contact-button events. The website code does not send form answers, names, email addresses, or phone numbers entered in the form to Analytics.
Your analytics choice is stored in this browser's local storage. You can change it with . Declining after previously allowing stops future analytics on the reloaded page and attempts to clear this site's Google Analytics cookies; it does not erase data already received by Google. Other browsers or cleared browser storage may require a new choice.
The site uses first-party browser storage to remember your privacy choice and, for the current session, whether an inquiry was submitted and whether you selected in-home or virtual. It does not save your form message or contact details in browser storage. Fonts and images are served from the website rather than loaded from third-party font services.
Keeping information and your requests
Inquiry emails may be retained as needed to respond and coordinate services, or to meet applicable recordkeeping obligations. Service providers have their own retention practices. You can request access, a correction, or deletion of inquiry information I hold by contacting me below; requests are subject to any applicable legal retention requirements.
